All forms must be filled out before being accepted at Erindale College either via digital or physical

Student Services (Eknex)

and upload the attached forms underneath that heading with the following blurbs:

Advance Notice of Absence – to be completed for an absence of more than 3 days.  Students will need to see each of their teachers, get a parent to sign the form and have their leave approved. Once the form is complete it should be lodged at eknex

Change of Personal Details – Please email from an authorised email account all change requests. Please note  - Proof of residency is required for any change of address (i.e. licence, rates notice or bank statement)

Students requiring additional support – (complete if applicable only)

Medical information and Consent form -  must be completed by the parent/carer of each student in an ACT public school. The form is used to request general medical information about students and provides for parents/carers to consent to first aid treatment for their child/ren in line with the directorate’s First Aid Policy and First Aid General Procedures. This form must be completed annually. Parents/carers must inform the school immediately if there are any changes to the information provided within the form

Known Medical Condition Response plan -  this plan is required for any student with a known medical condition, short or long term, that requires intervention i.e. the administration of medication or other support; and/or could lead to a medical emergency.

USI information on how to apply and form – to participate in vocational courses e.g. White Card, RSA while at college students will need to get a USI.  Students may also need to provide evidence of their vocational achievements when applying for a job or to undertake further study so it is important that all students apply for a USI. A student must have a USI prior to a vocational qualification or statement of attainment being issued.

Chromebook Acceptance form –  If you wish for your child to participate in the Better schools for our kids – technology enabled learning initiative, you and your child both need to complete and sign Part A and Part B of this form without changing it.
